血栓症は、PNHの主要な死因の一つです1-3
慢性的に溶血が起こっているとき、過剰な遊離ヘモグロビンは血漿中の一酸化窒素を減少させます。これにより、血小板の活性化や線維素溶解の障害が起こり、静脈や動脈での血栓症リスクが上昇します1-4。

血栓症（静脈性・動脈性）は、PNHに関連した死因の約40～67％を占めます5。
- 初発の血栓イベント（TE）が、生命にかかわることがあります6。
- 初発の血栓イベントは、死亡リスクを5～10倍増加させます5。
- 肺塞栓症（PE）や深部静脈血栓症（DVT）は非常によくみられる臨床所見です7。
- 血栓イベントのうち15％は、動脈循環で確認されています5。
PNHクローンの大小によらず、すべてのPNH患者さんにおいて血栓症のリスクが考えられます6。
- PNHクローンサイズの増大に伴い血栓症のリスクが上昇する一方で、PNHクローンサイズが小さい患者さんにも血栓症が発生すると報告されています6
- PNH患者さんの60％に、何らかの血栓症エビデンスが存在するという報告があります8。
血栓症は、溶血がごくわずかでも、輸血歴が無くても、高度の貧血がみられなくても起こることが確認されています5。
